On this page you will find the candidate reference form as well as a link to our online credential file partner, Interfolio. If you have come to this page to fill out a reference form for a candidate please follow the directions below under the Reference Form heading. If you have come to this page to login to your Interfolio portal please click on the link under the Interfolio heading.
Reference Form
Below you will find a document called Reference Form, it is a downloadable document. You will need to save the blank form to your computer, open it, fill it out, save it, and follow the directions provided by the candidate for which you are filling it out.
