Your Employer has selected Jefferson Pilot Financial Life/AD&D, Optional Life/AD&D and Long Term Disability for the employees of MidAmerica Nazarene University

Eligibility: All active full-time employees working 30 hours or more per week.

LIFE/AD&D (provided by your employer)

• One times annual salary, rounded to the next higher 1,000, subject to a minimum of $15,000 and a maximum of $50,000.

• The amount of Life Insurance and AD&D will reduce:

* 35% upon the person's attainment of age 65
* an additional 25% of the original amount at age 70
* an additional 15% of the original amount at age 75
Benefits will terminate upon retirement

Optional Life/AD&D
(available through payroll deductions)

• Coverage in increments of $10,000 to 7 x annual salary or $200,000.

Long Term Disability
(available through payroll deductions)

• Benefit percentage of 60% to a 45,000 maximum monthly benefit
• 90 day elimination period
• Maximum benefit period to 65/SSNRA
• 24 month own occupation
